• SURVEY forms for AMS - on the www.ams.org website. These need filled out occasionally throughout the year. This is where you can usually find the most up to date forms and also find information on where or who to send them to.

• TUITION Fee Waiver Forms - These are no longer filled out in paper form... to file for a tuition waiver you must:
           - Log on to FlashLine at www.kent.edu
           - Select the My Action Items tab
           - Find the Workflow and Utilities channel (box) on the top right corner of the screen.
           - Go to the Workflow section and click on Tuition Waiver Benefit Request; follow the directions on the form to submit your request.

Technotrash Recycling - The Libraries have partnered with GreenDisk to offer recycling services for technotrash. Technotrash cans are available in the Information Commons and the Computer Lab, both on the first floor of the Main Library. Staff may also use the can in the Technical Services dept on the third floor. If you are interested, drop off any of the following for recycling and environmentally-friendly disposal:
